A helo is several thousand parts moving in loose formation at the best of times.
I'm not a helo pilot, but I've read a bunch of the aerodynamic stuff...my guess would be some kind of engine failure where they were too low/slow to autorotate. As I understand it, in order to autorotate you need to have either speed or altitude to trade for temporary lift to cushion your descent.
I'd equate it to flying a twin at below Vmc
Again, not a helo pilot.
